The patient is a 61 year old Asian woman resident in the United States. She is mildly febrile with a cough productive of copious blood stained sputum. Clinical examination is unremarkable.

 

 
  1. Describe the major abnormality.

  2. List three possible causes.

  3. List three further investigations that are likely to lead to a firm diagnosis.

  4. What further points in the history would you like to elucidate and why?

  5. List three ways in which the radiographic technique used for this film could have been improved.

 

 
 

Case2

A 37 year old woman, known to be hypertensive but previously well and taking no routine medication. She has been found unconscious in her cabin. Relatives report that the previous evening at dinner she had complained of occipital headache and nausea and had retired to bed early.

 

 

 

 

  1. Describe the chest film findings.

  2. Give three ways in which the radiographic findings might be related to a neurological cause for her coma.

  3. How might a previous record of isolated third nerve palsy be related to the immediate history and the chest film.

  4. In the absence of this pathology, the film would have been over-penetrated. How would you improve it?
